back motor mount keeps coming off. new motor and ...

back motor mount keeps coming off. new motor and motor mount installed. nothing is binding or stuck. when first installed/ realigned works good initially and then starts to make a noise ( fan hitting casing) until eventually the mount comes loose/ off and the fan jambs the motor.
